COMMpanion offers a convenient Primary Care solution, powered by Penn State Health. It provides direct primary care services that are available whenever and wherever you may need them. Starting from your initial appointment, our healthcare professionals will tailor a customized plan to optimize your health. With a focus on user-centric care, COMMpanion's dedicated team offers a range of services including:

 <li>Well visits</li>
 <li>Urgent care</li>
 <li>Care navigation</li>
 <li>Messaging with a nurse</li>
 <li>Prescription refills</li>
 <li>Health coaching</li>
</ul>
Through COMMpanion, you establish a strong relationship with a primary care provider (PCP), similar to a conventional family practice setup. The unique aspect is the inclusion of an app that grants you access to a comprehensive team consisting of a nurse and a health navigator. The process is straightforward:
<ol>
 <li>We'll pair you with a PCP for your initial consultation.</li>
 <li>Based on your medical background and health issues, your PCP will formulate a tailored healthcare strategy for the upcoming year.</li>
 <li>Your health navigator will contact you regarding scheduling tests, vaccinations, and other appointments at one of Penn State Health's facilities.</li>
 <li>You can book sick and well visits as needed, with the option of up to three appointments per month.</li>
 <li>You will receive reminders for prescription refills, which can be delivered to your preferred pharmacy or directly to your residence.</li>
</ol>
If you have non-urgent inquiries, you can easily communicate with your team through the COMMpanion app or website and expect a same-day response.
For urgent concerns, urgent care video visits with qualified providers are available around the clock as some healthcare needs cannot wait for an in-person visit. Additionally, every enrolled family in COMMpanion receives a home examination kit for monitoring vital signs such as temperature and blood pressure in real time.
Care should be accessible whenever required. The COMMpanion app ensures consistency in care availability from an adept team familiar with your well-being and committed to maintaining your health.
If you are an Apple Health user, you have the choice to share vital health data with your provider during video visits. While optional, sharing Apple Health data like heart rate and weight can aid providers in delivering more accurate diagnoses.
